New Clients

Please let us know more about your pet's history by completing our new patient form prior to your first visit. You may complete the form by:

• Online Form: create an online account and complete our New Patient form.

• Download Form: download and complete our new patient form. Bring the completed form to your appointment or email the form to RaritanAH@yourvetdoc.com prior to your pet's appointment.

*We will be happy to contact your previous veterinarian to obtain any necessary information or documentation regarding your pet's medical history.
